What to bring to the office
If you are coming for a medical visit, bring any old medical records and images that you
have (if this is your first visit), your Health Insurance card, and a picture ID. Also come
prepared to pay your co-pay with cash, check, money order, or credit card (Visa and
MasterCard only). Be sure to specify who referred you to this office when you fill out the
patient information form and sign it on all three signature lines.
If you are an established patient, be sure to update the office if your insurance or
demographics have changed.
